DETERMINING CANDIDANCY FOR BREAST AUGMENTATION WITH BREAST LIFT

Patients pursue a combination of breast surgery for a variety of reasons ranging from a desire to reform their post-pregnancy figure, to re-sculpting the physique after major weight loss. Generally, a good candidate for the dual procedure has sagging deflated breasts with nipples that face downwards and dip below the breast crease. Breast symmetry may be compromised as well. Clients anticipating the procedure should be physically and mentally healthy and have appropriate expectations based on the information gleaned during their in-depth consultation with Dr. Tzamantaki.

While Dr. Tzamantaki always strives to fulfill her patient’s wishes and goals in every way, she may discourage some people from doing a breast augmentation and advices them to do a breast lift. Clients who wish to be pregnant in the future or planning to lose weight should hold off, until a later date. Additionally, capsular contracture in patients with history of breast augmentation should be addressed before undergoing any revision implant surgery.

Patients with serious medical issues such as hemophilia, diabetes or hypertension may find themselves at higher risk for complications from combined breast surgery.

Lastly patients that smoke, are encouraged to quit it, at least one month before attempting a breast augmentation with lift, as this habit can create potential complications, including poor wound healing, infection and reduced blood flow.

WHAT TYPE OF BREAST LIFT INCISIONS?

STANDARD ANCHOR LIFT: The doctor will excise skin and fat through an incision shaped like an upside down “T”. The incision encircles the nipple area(areola), then runs down the breast vertically and finally horizontally along the breast crease. This approach, similar to the one used during breast reduction is the best option, suited for those with more severe breast ptosis (sagging) and nipples that hang below the bottom of the breast, as it affords the doctor adequate access for resculpting.

LOLLIPOP LIFT: During the lollipop lift the doctor creates a vertical incision running from the nipple area(areola), downwardly towards the breast crease. She can then recontour the angle of the breast and adjust the nipple to a more attractive position.

BENELLI LIFT: Also referred to as a donat lift, the Benelli lift requires only a small incision around the nipple area(areola), which is hidden by its dark pigment. This technique is usually chosen for patients with mild breast ptosis, smaller cup sizes and for those whose sagging is really mild.

WHAT ABOUT IMPLANT PLACEMENT?

There are two traditional placements for the breast implant: the first is subglandular, meaning on top of the pectoralis muscle and the second is submuscular, meaning underneath the pectoralis muscle. Most surgeons tend to favor the second option, as it offers a lower risk of capsular contracture, sustained shape and fewer instances of implant rippling. However, a discussion of the best placement is an important component of your consultation, during that time Dr. Tzamantaki will make her best recommendation for your lifestyle, body type and goals.

WHAT CAN I EXPECT FROM THE SURGERY?

Patients that will undergo a breast augmentation and a breast lift at once, can expect to be in surgery for one to three hours, depending the complexity of their procedure. Undergoing general anesthesia, the patient will have to spend the night at the hospital.

Breast augmentation combining a lift is often part of a Mommy Makeover. During this extended combination treatment, the patient may also undergo liposuction of the tummy tuck and facial rejuvenation. WHAT WILL RECOVERY ENTAIL?

Soreness, tenderness and discomfort in the breasts is expected after a combination of surgeries, but these symptoms can be dealt with pain medications, which Dr. Tzamantaki will prescribe you. Nowadays, the pain and discomfort after a cosmetic surgery, means that your doctor did not give you the required post surgery attention! Your bandages will be removed at the second post-op day and you will wear a special support garment that facilitates a faster healing and helps the breasts keep the new shape during recovery. Most of the accompanying swelling, will be dissipated within one to two weeks.  Patients can return to their daily routine and exercise three weeks post-op, but the majority of my patients return to their work within three to four days.
